Online Application for Enrollment

Please fill out the online application, and make sure you fill in each field. Please note that your application will not be complete until we have received all required documentation. When submitting an application, we will need the following information to establish the requirements for enrollment: Age verification: (Copy of child’s Birth Certificate or proof of live birth); Proof of Income: (We need at least one of the following documents – Tax Forms, W2, 1040, etc., Past 2 months consecutive pay stubs, or Signed letter from Employer stating salary.)
After enrollment, the following information is needed: Copy of Child’s most current Immunizations, Copy of Medical Card, Insurance Card, and/or CDIB card, Completed Well-Child Check or Physical Form, Dental Appointment, and Paperwork on Child’s Disability, if applicable.

Head Start and Early Head Start programs are free, federally funded programs designed to promote school readiness for infants, toddlers, and preschoolers from families that meet eligibility requirements.

Early Head Start programs support pregnant women and families with children younger than age 3 years. Head Start programs serve children between 3 and 5 years old.

Head Start programs also support children with identified needs (such as physical and developmental delays), children in foster care, and children experiencing homelessness.

The programs encourage family involvement through regular visits to the child’s home and provide opportunities for families to volunteer in the programs and participate in special activities to support their child’s learning.

Head Start and Early Head Start programs link children and families to other services in the community, such as free medical and dental care services.

We use an evidence-based learning curriculum and authentic child assessment tools.

We focus on the whole child and family to ensure future success in school.
